This product is intended for Group Cycle, Group Choreography, Group Conditioning, Mind Body, Aquatic Fitness and Fitness for the Older Adult Leader Certifications.
Reprints for Fitness Alberta Certification and Insurance Certificates are available upon request, fees apply for each
I understand that my order must be verified and processed before my certification is complete
I understand there are no refunds for recertification orders
I understand that my recertification cannot be submitted without complete documentation, including petition applications. I verify that I am submitting completed documentation including evidence of 12 Continuing Education Credits, current CPR issued within the last 12 months, and any required Practical Assessments.
I understand that recertification processing can take up to 4 weeks to complete and are processed in the month they are due.
Fit Rendezvous or Perspectives conferences or Fitness Alberta webinars *write dates attended in the Additional Notes section at the end
Fitness Alberta Approved Event *upload proof of course completion with course reference number (i.e. CEC 23045)
Fitness Alberta Specialty or Designation Course *include proof of course completion or write Course Reference Number and dates attended in the Additional Notes section at the end
Non Fitness Alberta Event Approved by Fitness Alberta Office *include letter of approval from Fitness Alberta office
Request to add facility information to insurance certificate *Full facility name AND mailing address required*
If requested by employer, the following MUST be added in the box below: Facility Name and Mailing address of the facility. Incomplete information will void insurance coverage.
CPR certificate
CPR must be issued within the last 12 months to be valid. Online CPR is not accepted.
Practical assessment
If this is your first recertification for any Specialty, please upload your practical assessment document(s).
Continuing education credit verification documents
Include all documents of related continuing education being used towards your recertification. For Fitness Alberta webinars and conferences, please only list dates attended in the Additional Notes section.
