This product is to complete and pay your fees for Fitness Alberta Personal Training Certification.
Reprints for Fitness Alberta Certification and Insurance Certificates are available upon request, fees apply for each.

CPR certificate date of issue
Must be issued within the last 24 months to be valid.
Request to add facility information to insurance certificate *Full facility name AND mailing address required*
If requested by employer, the following MUST be added in the box below: Facility Name and Mailing address of the facility. Incomplete information will result in the additional insured not being added to your policy.
Practical Assessment documents
